School Entry Requirements 

Sports Physical: Any student who will be in grades 7-12 for the 2024-2025 school year, and wishes to participate in extra curricular athletics must have a current sports physical done after May 1, 2024 as directed by the Kansas State High School Activities Association.

Immunization record: Please bring a copy of your child's immunization record with you when enrolling, if you are new to the district or have had any updates.

Administer Medicine: All medicines distributed at an Abilene School must have a Request to Administer Medicine form on file.

School Entry Physicial: All students between the ages 5-8 who is enrolling in a Kansas school for the first time must also present a completed physical examination that has been done in the previous 12 month period.

Food Allergy/ Modification: Any child that has a food allergy/intolerance or other dietary needs, will need to have a dietary form on file.  This form needs to be filled outeach year and signed by the child's physician.
